I, the undersigned, voluntarily consent to having my photograph taken and/or my voice recorded at Creekside Campground and Events Center. I grant SAWTOOTH RIDGE RANCH, LLC dba CREEKSIDE CAMPGROUND AND EVENTS CENTER ("Creekside") full permission to use my image, voice, likeness, and physical surroundings without restriction for promotional, publicity, marketing, or organizational purposes, including but not limited to: print materials, digital projection, internet, website, video, and any future media formats.
I expressly release SAWTOOTH RIDGE RANCH, LLC, dba CREEKSIDE CAMPGROUND AND EVENTS CENTER, its representatives, agents, and any institution transmitting or exhibiting my picture or voice, from any and all claims arising from such use or distribution.
I agree to be fully responsible for my own participation and hold Creekside Campground, its owners, employees, and representatives harmless from any liability, loss, or expense arising from the authorized use of my image or voice.
I also consent to the use of my name, voice, and/or picture, and other material about me, for promotional, publicity, or organizational purposes.
